The Order Form Web Part, enables you to use a any SharePoint List with categories and subcategories columns to be shown in a filtered way to the end users, so they can choose a category and and the subcategory dropdown values are filtered automatically.
Sample: You have product list, with the columns categories and subcategories: a columns prices, and description and image of the product: used as a shopping list/catalogue, users can access the custom form, select from the first dropdown the product. The second dropdown will than automatically filter the values for the subcategory field. Description another information's will be shown.
